Applications
- A description of the proposal including costs, use of funds, funding resources secured for this project, the specific amount requested from the Foundation and the reason for this request. The amount requested from the Foundation should appear in the first paragraph of the concept paper in bold & underlined font.
- Background information on your Club including: number of staff, number of youth served, current programs offered, a 3 year historical overview of the Club’s Operating Budget (highlighting funding categories and amounts received per annum) and a brief synopsis of Board leadership & activity.
- Please be sure the first page of your concept paper is on your organization's letterhead and that proceeding pages are footnoted with the page number, your organization's name, address and the date of submission. Please ensure all addendums are labeled with your organization's name.
- Ensure the concept paper is signed & submitted by the CPO/Executive director of your organization.
AND
- Please include a copy of your most recent IRS 501(c)(3) designation letter.
Concept papers will be reviewed by our staff on an on-going basis. Once all documentation has been received and the staff has completed its review, full applications are considered in chronological order by the Board of Directors. Grant requests are reviewed several times throughout the year.
While all requests cannot be funded, each qualified application is considered carefully and seriously. Decisions are made based upon the Foundation’s priorities and the amount of money available for distribution. **Successful grantees will be expected to provide program evaluations including impact reports as well as an accounting of how funds are spent.
The Pacific Youth Foundation only approves 1 yr. grant requests. The average grant from the Foundation is $40,000.
With respect for our grant process and stream lining our operations we request that only Executive Directors/CPOs contact the Foundation. We would also like to request that after a successful grant application organizations wait one year before applying for another grant. Our next deadline for grant requests is January 15, 2012.
